About the role

The Pediatric Nurse II acts as the clinical leader of the care team, managing high-risk and chronically ill patients while providing education and direct care. Responsibilities include triage, communication facilitation, and coordination with home care services.

Description Summary

The Pediatric Nurse II is the clinical leader of the care team. Under the Supervision of the Pediatrician, Nurse Practitioner and Nurse Manager, the Care Team Nurse maintains smooth flow of operation, manages high risk and chronically ill patients, provides education to patients and families, provides direct care and laboratory services and is responsible for telephone and direct triage.

Job Responsibilities

Demonstrates basics of infection control including standard precautions Demonstrate skills and knowledge directly related to age-appropriate care of pediatric patients as well as proficient knowledge of equipment and instrumentation Assess the physical, emotional, psychosocial and socio-economical needs of the client and family, and refer to appropriate organizations for further evaluation when appropriate Provide urgent triage care in the office as needed as well as urgent telephone triage and/or crisis management Use EMR to completely and accurately documents nursing care including triage, education, medication, reconciliation, and care planning when appropriate Act as facilitator of communication between family, physician, front desk and other health professionals Administer vaccines and medications, and perform in-office procedures such as blood work and catheterization in an efficient and professional manner, delegate appropriate tasks to Medical Assistant and other staff when appropriate Contact patients and families after Emergency Room ad Hospital discharge to prevent readmission and post-hospitalization complications. Coordinate with home care services if necessary Maintain pediatric-centered continuing education and training Adhere to the general policies and procedures as outlined by the practice Ability to meet the necessary physical demands of the position

Requirements

Other Qualifications: 2+ years nursing experience Competent in the procedure of catheterization Excellent written and verbal communication, excellent organization Ability to work independently as well as function as part of the care team Ability to handle sensitive medical information/HIPAA compliance Working knowledge of EMR, Microsoft office and email Educational Requirements: Graduation from an accredited nursing program. The nurse requires a current license to practice nursing in the Commonwealth of Massachusetts by the Department of Public Health Nursing Certification Board as either a Registered Nurse (RN) or Licensed Practical Nurse (LPN). The nurse must also maintain Basic Life Support (BLS) certification and Continuing Educational Units (CEU’s) as required for licensure. S/He must also practice within the guidelines of the American Nursing Association Standards or Practice.
