The Registered Nurse provides home health services to help patients achieve the highest possible level of independence and well-being. They are responsible for managing disease processes and preparing patients and families for a continuum of care.
The goal of the home health Registered Nurse (RN) is to return the patient to the highest practicable level of independence and well-being within the community with self-management of disease and other identified processes. When the highest practicable level of well-being is not independence, the goal is to prepare the patient and family/caregivers for a continuum of care within the home and/or within other community-based health care systems.
Candidates must be a graduate of an accredited nursing school with an active Colorado license and at least one year of RN experience. A valid driver's license and the ability to communicate in the client's language are also required.
