The RN Case Manager will manage a structured caseload of up to 18 patients and build strong, supportive relationships with patients and families. They will collaborate with a team that values communication, accountability, and compassion.
The ideal candidate is a compassionate and patient-centered RN who is reliable, organized, and able to manage responsibilities independently. They should be comfortable with travel throughout the assigned territory and committed to providing thoughtful hospice care.
