Social worker ii– master’s level (Full-Time – 35 hours/Week)

Shift Schedule: Monday–Friday, 9:00 AM – 5:00 PM

As a Social Worker II, you will provide expert clinical case management and crisis intervention services to older adults. Utilizing your advanced training, you will conduct in-depth assessments, develop complex care plans, and advocate for essential government benefits to help mentally and physically impaired seniors maintain their autonomy and quality of life.

Responsible for the clinical oversight of a diverse caseload, you will ensure that every client receives a personalized, holistic approach to care.

• Triage: Perform preliminary screenings and emergency triage; conduct comprehensive In-Home and In-Office assessments using standardized tools (e.g., PHQ-9, NSI, STARS).

• Planning: Develop and implement written case plans that include individual and family counseling to resolve complex social and emotional challenges.

• Coordination: Arrange for critical supports such as home-delivered meals, home care, Medicare/Medicaid, SNAP, SCRIE, and psychiatric therapy.

• Protection: Evaluate clients' capacity to manage their affairs; assist in securing financial management or guardianship to prevent exploitation.
