Provide information, referrals, and advocacy services to older adults to help them access critical health and social resources. Manage case records and conduct community education workshops to promote independence and stability for participants.
Requires a bachelor's degree in Social Work or a related field, or equivalent work experience. Must possess strong communication skills and the ability to work flexible hours for emergency operations.
Case Manager – Older Adult Centers As a Case Manager, you’ll provide information, referrals, advocacy, and case assistance services to older adults participating in JASA’s Older Adult Centers in Queens. Through in-person, you’ll help connect participants to critical resources and services that promote independence, stability, and overall well-being.
