Under the direction of a Master’s-level supervisor, you will provide critical crisis intervention, protective services, and social casework for older adults who are mentally or physically impaired, abused, or neglected. This role operates on both a voluntary and involuntary basis, requiring a high level of professional judgment and a commitment to elder justice.

📝 what you’ll be doing

You will manage a complex caseload, performing everything from emergency triage to long-term protective planning.

• Intake: Conduct preliminary screenings and triage for emergency interventions; provide immediate information and referral services to clients and families.
• Comprehensive
• Assessment: Perform in-home and in-office assessments of social and emotional needs using standardized instruments (e.g., PSA, EISEP-PATH).
• Case
• Planning &
• Implementation: Develop written case plans including individual/family counseling, benefit screening, and arrangement of services (Home Health Aides, Legal Aid, Housing, etc.).
• Financial &
• Legal
• Protection: Evaluate clients' capacity to manage their affairs; assist in securing financial management or guardianship to prevent elder abuse and financial exploitation.
• Field
• Monitoring: Conduct ongoing in-home visits to monitor risk, evaluate functioning, and revise case plans in real-time.
