The Licensed Practical Nurse (LPN) provides outpatient nursing care, supports medical staff in delivering preventive care, and manages chronic disease populations. This role includes patient education, coordination of care, and participation in quality improvement initiatives.
Candidates must be graduates of an accredited nursing school and hold a current Washington State LPN license. Strong documentation skills and knowledge of nursing practices are essential, along with the ability to communicate effectively with diverse patient populations.
The LPN’s primary focus includes coordination of care across the continuum and chronic disease population management, while also providing patient education under the supervision of a medical provider. This position also participates in quality improvement initiatives, clinic operations, and may oversee students and allied health professionals during their educational experiences. Essential Functions Maintain efficient patient flow, including taking vital signs and rooming patients Perform nursing procedures, therapies, tests, and services within scope and as ordered Promote a safety-conscious work environment Assess, plan, implement, and evaluate age-appropriate patient care Communicate effectively with diverse patient populations Prioritize and manage patient care (in-person visits, calls, and written requests) Support provider schedules and ensure efficient clinic operations Coordinate patient care across services, providers, and care settings Assist in chronic care management (e.g., depression, diabetes, CHF, chronic pain) Administer medications, perform waived testing, EKGs, and treatments Prepare patients for exams and assist with procedures Educate patients and families on health topics under provider supervision Monitor and report patient conditions and responses to treatment Obtain and document patient histories Maintain accurate patient records and perform data entry Handle patient calls and provide treatment guidance as appropriate Practice within scope of license and clinic protocols Adhere to all JST and JFHC policies, including safety and HIPAA standards Perform additional duties as assigned Requirements Graduate of an accredited school of nursing Current Washington State LPN license Current First Aid/CPR certification Knowledge of nursing theory, regulations, and patient care practices Understanding of safety, infection control, and environmental precautions Strong documentation and record-keeping skills Demonstrated clinical competence Ability to maintain high standards of nursing care Excellent interpersonal, verbal, and written communication skills Basic computer proficiency Valid driver’s license Ability to pass a criminal background check
