As a Community-Based Therapist, you will provide direct clinical services and support to individuals and families in their homes, schools, and other community settings. Responsibilities include:

• Clinical Services Conduct intake procedures, including all required forms and releases.
• Provide diagnostic assessments to determine eligibility and treatment needs.
• Facilitate individual, family, and group therapy using culturally responsive, evidence-based approaches.
• Deliver crisis intervention, safety planning, and risk assessments when needed.
• Treatment Planning & Collaboration Develop collaborative treatment goals with clients and families.
• Identify client strengths, needs, and barriers to inform the Individual Service Plan (ISP).
• Modify treatment plans as needed and communicate progress during treatment team meetings.
• Coordinate care with community partners, schools, and other service providers.
• Documentation & Compliance Complete treatment plans, progress notes, and discharge summaries in a timely manner.
• Maintain productivity expectations and meet all state, federal, and funding requirements.
• Participate in clinical supervision and ongoing professional development.
• Community Engagement Connect clients to community resources and ensure follow-up on referrals.
• Support clients navigating trauma, addiction, grief/loss, family conflict, chronic stress, and other challenges.
• Provide services where clients feel most comfortable—home, community, school, or telehealth.
