The Case Manager facilitates individualized treatment plans, coordinates transitions of care, and provides clinical support for adults with substance use disorders. They also act as a community liaison, managing referrals and ensuring compliance with regulatory standards and electronic health record documentation.
Candidates must hold a Bachelor’s degree in behavioral health or a related field and possess at least two years of case management experience. Additionally, one year of experience working with Alaska Native/American Indian populations is required, along with proficiency in trauma-informed care and substance use disorder treatment models.
This position facilitates a seamless entry into services advocates through treatment and navigates complex service systems for consumers at the Ralph Perdue Center South, a 24/7 ASAM (American Society of Addiction Medicine) Level 3.1 clinically managed low-intensity residential treatment program serving adults with severe substance use disorders and co-occurring conditions. As these consumers may have faced significant stigma, trauma, and social, economic, and structural barriers to accessing treatment, this case management role is proficient in understanding substance use and assisting consumers in learning coping skills and creating stability in their lives ensuring a seamless transition through the full spectrum of care, bridging high-acuity stabilization and clinical progress as consumers move toward community reintegration and long-term recovery.
