The Case Manager coordinates patient care across the continuum from admission to discharge to ensure safe transitions and optimal patient flow. They collaborate with multidisciplinary teams to resolve barriers to care and manage the patient's plan of care.
Candidates must hold a Registered Nurse license or a Social Worker license with at least one year of relevant healthcare experience. A Bachelor's degree is required or must be obtained within three years of hire.
The Case Manager (CM) I works in collaboration with the patient/family, physicians, and multidisciplinary team members to ensure patient progression through the continuum of care and to independently develop a plan of care for each assigned patient from admission through discharge with minimal direction. Responsible for identifying, initiating and managing optimal patient flow/throughput to enhance continuity of care, smooth and safe transitions, patient satisfaction, patient safety, and length of stay management. Utilizes their knowledge and expertise to evaluate the individual patient's needs via comprehensive assessment and managing care coordination and discharge planning. The CM assesses and responds to patient/family needs by coordinating efforts of the interdisciplinary team members and identifies and resolves barriers that hinder effective patient progression of care. Ensures all communication is appropriate, accurate, timely, and consistent with the patient’s plan of care.
Education: BSN or MSW with licensure and 1 year of healthcare setting discharge planning, care coordination, or OSF Case Management Internship Licensure/ Certification: Clinical Social Worker License (LCSW) or Licensed Master Social Worker (LMSW) or Clinical Professional Counselor License (LCPC) in IL or MI OSF HealthCare is an Equal Opportunity Employer.
