The Care Coordinator manages and facilitates person-centered care plans for individuals with intellectual and developmental disabilities. They act as a primary liaison between healthcare providers, families, and community resources to ensure access to necessary services.
Candidates must possess a bachelor's degree in a social services field or be a licensed nurse with relevant experience. A minimum of two years of experience working with individuals with intellectual and developmental disabilities is required.
