The Clinical Social Worker will perform assessments, counseling, psychotherapy, and education for individuals and families. They will also provide referral services and manage care coordination or discharge planning in a medical setting.
Candidates must hold a clinical Social Worker license from any US state and possess at least two years of full-time experience in care coordination, discharge planning, or case management. A Master's or Doctorate degree in a relevant field is required.
