  • Summary: The Advanced Practice Provider (APP) plays a vital role in delivering specialized care and managing the health needs of patients in either inpatient, ambulatory, or a combination of both settings. With advanced training and expertise, the APP focuses on subspecialized care, including chronic illness and disease management, ensuring continuity of care throughout the patient’s healthcare journey, which may span multiple years. This role involves specialization in a specific organ system, where the APP provides expert consultation within their area of expertise as appropriate. The APP is integral to supporting patients across the continuum of care in both inpatient and outpatient specialty settings, collaborating with multidisciplinary teams to achieve optimal patient outcomes. .

  • Summary: The Hi-RiSE clinic is located at The Vanderbilt Clinic, 1301 Medical Center Drive, Nashville, TN 37232. We provide preoperative anesthetic evaluations, including surgical risk stratification, and develop and implement plans of care to optimize medically complex adult individuals in the weeks before they undergo elective surgery. We are looking for a motivated, self-directed advanced practice provider (APP) who is committed to patient care excellence within a collaborative and compassionate environment that promotes teaching and learning for both patients and team members. Each patient’s plan of care is initiated by the APP and supported by registered nurses and support staff. The leadership team is comprised of a medical director (who is also the APP collaborative physician), nursing director, and clinic manager. Clinic support staff is comprised of medical assistants and patient service specialists. The Hi-RiSE clinic shares a unique relationship with the Vanderbilt Preoperative Evaluation Clinic (VPEC) and team members are cross trained to work within both clinic systems. Various interventions and resources are utilized for patient care including laboratory testing, imaging, prescription of medications, collaboration with primary care provider, and medical specialty clinic referrals and testing. We are committed to providing excellent patient centered care while providing a supportive and caring work environment for our team members. Nurse Practitioner (FNP, ANP, AGPCNP, AGACNP) or Physician Assistant (PA) Prior APP Experience Strongly Preferred KEY RESPONSIBILITIES Obtains and documents a health history; performs and documents complete, system-focused, or symptom-specific physical examination, assessment and plan of care. In collaboration, provides healthcare services for primary, acute and complex care of patients and manages patients’ overall care, identifying expected outcomes for diagnoses. Orders, performs, interprets and collects data using appropriate assessment techniques, relevant supporting diagnostic information and diagnostic procedures where indicated. Forms differential diagnoses and treats acute and chronic conditions. Prescribes therapeutic interventions for both pharmacologic, non-pharmacologic and surgical, needed to achieve expected outcomes. Utilizes evidence-based, approved practice protocols in planning and implementing care. Initiates appropriate referrals and consultations Provides specialty specific consultation services upon request and within a specialty scope of practice.
