The Hospice RN Case Manager provides skilled nursing care and coordinates comprehensive care plans for patients in their final chapter of life. They collaborate with an interdisciplinary team to manage symptoms, support families, and ensure high-quality, compassionate end-of-life care.
Candidates must be a graduate of an accredited nursing program and hold a current Registered Nurse license. A minimum of one year of clinical nursing experience and reliable transportation for daily travel are required.

Bring your nursing expertise to one of healthcare's most meaningful specialties. As a Hospice RN Case Manager, you'll provide skilled nursing care and compassionate support to patients and families during life's final chapter, helping ensure comfort, dignity, and quality of life.
This role is perfect for a registered nurse who wants to practice the full scope of nursing while building deep, meaningful relationships. This position will have you building report with patients, families, physicians, facilities, and other interdisciplinary team members. You'll have the autonomy to coordinate comprehensive care for your caseload while working collaboratively with our interdisciplinary team to address the physical, emotional, and spiritual needs of those in your care.
Pay Range $80,000—$85,000 USD
Employment is contingent upon the successful completion of a background check. Screening is completed by a third-party administrator, the Michigan Long-Term Care Partnership, and is performed in compliance with the Fair Credit Report Act.
