The Adult Care Coordinator provides integrated case management, advocacy, and wellness services to clients within a multidisciplinary team. They are responsible for coordinating medical care, facilitating hospital discharges, and implementing suicide prevention safety plans.
This position requires a Bachelor’s degree in social work or equivalent education. Candidates must obtain Case Management II and Behavioral Health Wellness Coach certifications upon hire and possess a valid driver's license.
Certified Community Behavioral Health Clinic (CCBHC) provides integrated medical and behavioral health care to clients in an office-based setting.
Position provides advocacy, linkage, referral, engagement with ongoing services with a multidisciplinary team. Coordinating these services to clients in CCBHC program in a client-centered and culturally responsive way. Provides a point of contact and work in partnership with CCBHC staff and partners as part of a multidisciplinary team to provide services to clients and work within a team
Requires Bachelor’s degree in social work or equivalent education. ODMHSAS CMII certification preferred.
Must obtain Case Management II Certification and Behavioral Health Wellness Coach certification upon hire.
Must possess Driver License and use personal automobile for local travel. Must transport clients in personal automobile.
Safety Sensitive job classification
This job is classified as a "safety-sensitive" position as defined by the Oklahoma Medical Marijuana and Patient Protection Act.
