How you’ll make an impact in this role Reduce Readmissions & Ensure Safety
Oversee complex discharge plans, ensuring patients have crucial resources at home, such as housing, follow-up care, and medications. They tackle non-medical issues and address the root causes: referred to as Social Determinants of Health (SDOH)—like food insecurity, housing instability, and financial challenges that affect patients' ability to heal and adhere to treatment

Support: Deliver vital mental health counseling, offer crisis intervention, and connect patients to behavioral health services. Provide advocacy & resource expertise: Serve as a strong advocate for patients and families, coordinating various community resources to improve the patient's overall well-being post-discharge.
What minimum requirements you’ll need Licensure / Certification / Registration
Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted. Certified Social Worker preferred.
Education
Bachelor's degree in Social Work required.
Work
Experience: 3 years of experience preferred.
