Perform initial and ongoing patient assessments while developing and implementing tailored treatment plans in a home health setting. Collaborate with interdisciplinary teams and provide education to patients and caregivers to optimize health outcomes.
Requires a current physical therapy license, a degree from an accredited program, and valid BLS certification. Candidates must have at least one year of experience and a valid driver's license with access to a reliable vehicle.
