Job Summary The Integrated Social Worker (ISW) is a key member of our multi-disciplinary primary care and behavioral health team. The ISW partners closely with providers, community health workers, psychiatry providers, counselors, practice managers, and medical assistants to assist in the identification and triage of acute mental and behavioral health needs which arise within the primary care setting. The ISW will complete thorough diagnostic assessments to determine the appropriate mental health and substance use disorder diagnostic and treatment recommendations. The ISW serves as a patient advocate, collaborative team member, educator, and community health resource. Integrated social workers may also participate in process improvement, counseling, education, advocacy, and other duties as assigned based on patient census and clinical needs. Responsibilities and Duties RESPONSIBILITY 1. Professionalism and Communication Maintains high level of professional communication with both patients and team in both written and oral formats. Manages schedule and time management to meet monthly productivity goal. Demonstrates punctuality with patient appointments, meetings, or scheduled time in clinic. Consistently attends provider, behavioral health, and other meetings as requested by supervisor. Maintains charting in patient’s medical records with behavioral health diagnosis, evaluation, treatment plan and progress in compliance with Lower Lights policies. RESPONSIBILITY 2. Direct Patient Care Is a member of the multidisciplinary, primary care team, and communicates and interacts with all levels of staff, primary care providers and patients with professionalism and respect. Maintains a service-oriented approach as a member on a collaborative team. Provides crisis intervention consultation for LLCHC patients to ensure patient safety, assess patient needs, and provide recommendations regarding level of care. Performs psychosocial assessments for purpose of referral to behavioral health services. Demonstrates competency in choosing appropriate diagnostic tools, implementing these tools, completing a differential diagnosis, and accurately determining level of care required. Provides patient-entered education on mental health diagnoses and recommendations Provides recommendations to psychiatry and/or primary care providers as outlined by clinical workflows and meaningfully participates in multi-disciplinary care discussions as needed. Attends community and partnership meetings as required. Provides assessment and brief counseling (1-6 sessions) to patients, develops and implements individualized treatment plans Demonstrates competency in screening, assessment, and treatment of perinatal mood and anxiety disorders (PMADs) RESPONSIBILITY 3 Education and Licensure Maintains CEUs to meet state professional licensure requirements and maintains knowledge and skills in applying updated research, evidence-based practice, and application of best practice guidelines. Demonstrates willingness and initiative in sharing lessons learned or knowledge gained with peers and other providers Maintains good standing with the appropriate board and certification entities. Maintains professional competence by pursuing opportunities for continuous learning, attending continuing education offerings, reading professional literature, and engaging in other activities, which develop new knowledge and skills. QualificationsQUALIFICATIONS:
Bachelors or Master’s degree in social work or related field from an accredited university.
Current licensure including: LPC, LPCC, LPCC-S, LSW, LISW, LISW-S
Evidence of mature judgment, ethical conduct, and leadership skills.
Skill in crisis intervention and de-escalation strategies.
Demonstrates ability and fluency in appropriately selecting, administering, and interpreting validated screening tools used in behavioral health within a multi- disciplinary team environment.
Strong oral and written communication skills, including the ability to collaborate and communicate with both a diverse team and diverse patient population.
Ability to work independently and implement time management strategies.
Must be willing to work in multiple sites within the greater Columbus area as needed. Experience with trauma informed care and behavioral health or health care settings.
