The Health Coordinator supports client physical and behavioral health services by facilitating communication between providers and staff within residential facilities. They also provide direct client support, monitor medication administration, and maintain accurate documentation in electronic health records.
Candidates must have a high school diploma or equivalent and at least one year of experience in a caregiving or human services setting. Additionally, applicants must obtain Agency Affiliated Counselor registration and First Aid/CPR certification within 90 days of hire.

New hires for this position typically start between $17.30 and $21.28 hourly, depending on factors such as work location, experience, qualifications, skills, competencies, internal equity, and market conditions. While the full pay range is provided to support pay transparency, individual compensation will vary within the range based on these factors.

What you'll do
The Health Coordinator supports Pioneer Human Services’ mission by coordinating and monitoring client physical and behavioral health services within residential treatment facilities. This role assists in ensuring continuity of care by facilitating communication between clients, nursing staff (where present), medical and behavioral health providers, pharmacy services, and program staff.
The Health Coordinator supports the intake process, coordinates internal and external medical and behavioral health appointments (including telehealth), assists with medication monitoring and documentation accuracy in the electronic medication administration record (eMAR/MAR), and gathers and communicates relevant health information to appropriate clinical staff. At sites with licensed nurses, the Health Coordinator works under the direction of nursing staff to support care coordination and implementation of provider orders. At sites without nurses, the Health Coordinator works under the direction of Health & Resource Coordination (HRCU) Manager.
In addition to care coordination, the Health Coordinator provides direct client support within a structured residential environment, including; engaging clients in the milieu, observing and monitoring client changes in physical and in behavioral health status, reinforcing treatment plans and health education, assisting with access to healthcare services, and responding to urgent health concerns by notifying appropriate clinical staff or emergency services as needed.
This role requires strong organization, attention to detail, and the ability to effectively support clients with complex medical, behavioral health, and substance use needs in a structured residential environment.
Specific responsibilities related to medication handling and clinical coordination may vary by site or unit depending on staffing structure, program licensure, and the presence of licensed nursing staff.
