Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients. Measure vital signs, Administer IVs, change dressings, clean wounds as needed Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations. Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload. New York State RN license Minimum of one year of recent hands on experience as a Registered Nurse. Basic Life Support Certification Valid New York State Driver’s License and reliable transportation

Candidates must hold a valid New York State RN license and have at least one year of recent hands-on nursing experience. A Basic Life Support certification and a valid driver's license with reliable transportation are also required.
