What you will do
Presents program information to patients, families, or community groups in order to promote good health outcomes and recruit clients into programs.
Provides program awareness to community partners.
Enters data, documents and evaluates the effectiveness of program activities.
Follows up with community referral sources and potential clients for recruitment.
Performs continuous quality assurance calls with new and existing program clients.
Performs other job functions as assigned or requested.

Candidates must have a high school diploma or equivalent, with a bachelor's degree in healthcare or social services preferred. A minimum of 1-3 years of experience in the Nurse Family Partnership program is required.
