• Position Overview The Licensed Practical Nurse (LPN) or Registered Nurse (RN) plays a key role in providing compassionate, high-quality clinical care to our residents.
• This position is responsible for medication administration, monitoring resident health, communicating with families and providers, and supporting the caregiving team.
• The LPN works closely with the Executive Director and leadership team to ensure residents receive safe, person-centered care in accordance with state regulations and community standards.
• Key Responsibilities Administer medications and treatments in accordance with physician orders and state regulations.
• Monitor residents for changes in physical or cognitive condition and report concerns promptly.
• Maintain accurate and timely clinical documentation.
• Assist with resident assessments and care plan updates as needed.
• Communicate effectively with families, physicians, pharmacies, and outside healthcare providers.
• Support and guide caregivers and medication aides during shifts.
• Respond to resident health concerns and emergencies appropriately.
• Ensure compliance with assisted living and memory care regulations.
• Participate in care conferences and family communication when needed.
• Assist with training staff on proper care techniques and dementia care best practices.
• Promote a positive, respectful, and team-oriented work environment.

Qualifications

Licensed Practical Nurse (LPN) or Registered Nurse (RN) in good standing. Experience in geriatrics, assisted living, memory care, or long-term care preferred. Knowledge of medication administration and documentation practices. Strong clinical observation and communication skills. Compassion, patience, and a genuine passion for caring for individuals living with dementia. Ability to work collaboratively with caregivers, leadership, and families. Working Environment This role operates within a memory care residential setting and involves frequent interaction with residents, families, staff, and healthcare providers. Light lifting may occasionally be required.
