The LPN provides skilled nursing care to patients in their homes and maintains accurate electronic medical records. They coordinate care with the home health team and report patient status changes to the Registered Nurse.
Candidates must hold a valid LPN license in good standing and meet all federal and state licensure requirements. Basic knowledge of electronic medical records and current CPR certification are also required.
The LPN is responsible for traveling to a patient/client's place of residence to provide skilled nursing care. The LPN is responsible for maintenance and accuracy of electronic medical records and coordination of care with other team members. The LPN works under the supervision of the Registered Nurse (RN) as a member of a team to give direct nursing care to patient/clients within the policies of the agency. The LPN will provide services within the scope of Federal and State regulations, licensure, policies and procedures of the agency.
LPN in good standing with discipline specific licensing/certification agency and able to meet all federal and state licensure requirements. Current CPR certification or ability to obtain within 90 days of hire.
Experience Basic level knowledge of EMR (electronical medical records) and email functions.
