The Care Management Support Specialist provides support to the Care Management Program Supervisor by assisting enrollees with complex behavioral and chronic physical health conditions. This role involves collaborating with health plans and providers within the Network180 system of care.
Candidates must possess a high school diploma or GED and hold or be eligible for a State of Michigan Certified Peer Support Specialist or Peer Recovery Coach certification. A valid driver's license and access to reliable transportation are required for community-based work.

The Care Management (peer) Support Specialist provides support to the Care Management Program Supervisor, supporting enrollees with complex behavioral and chronic physical health conditions, in collaboration with one or more health plans and providers in Network180’s system of care.
00 am to 5:00 pm. There are no weekends, on call, or holiday requirements for this position. This assignment is in person at 790 Fuller Ave Grand Rapids, Michigan 49503 and within the community.
