Our mission is to help patients maintain their independence while reducing unnecessary hospitalizations and emergency room visits.

Our team of nurses and healthcare practitioners works closely with patients and their families during periods of illness and recovery. Together, we collaborate on individualized plans of care that bring the right services directly into the home.

Qualifications

Current Nebraska RN or LPN license in good standing DD Waiver experience preferred Ability to pass a background check (criminal history, APS/CPS) Strong communication and documentation skills Physically able to assist with transfers and lift 50+ lbs
