The LPN will provide direct patient care and collaborate with the interdisciplinary team to develop and implement treatment plans. They are responsible for observing, evaluating, and documenting patient outcomes while providing clinical information to the RN.
Under the direction and supervision of an RN/GN, the LPN/GPN is expected to utilize his/her knowledge of practical nursing, age-specific needs, the care/treatment of the Rehab patients. The LPN/GPN is to be an active member of the interdisciplinary treatment team and work collaboratively in the development, implementation and revision of each patient’s interdisciplinary treatment plan. The LPN/GPN is to observe and evaluate each patient, document his/her observations and evaluations and provide the RN with relevant clinical information.
Must be a graduate of an accredited LPN program.
Minimum Experience Required: Prefer 1 year of medical surgical experience preferred
A current license as a Licensed Practical Nurse in the Commonwealth of Pennsylvania. Must be BLS certified required (or obtain within one month of hire).
A commitment to service excellence and performance improvement. Must possess nursing knowledge of medical, surgical and behavioral health nursing to effectively provide direct patient care, observe and document outcomes. Provide patient and family education. Excellent writing, computer and interpersonal skills. The ability to work collaboratively as a member of the interdisciplinary team.
Shifts: Rotation/Mainly Nights
