About the role

Coordinates the provision of direct patient care by assessing, planning, and implementing nursing care plans. Collaborates with the healthcare team to manage patient rehabilitation, discharge planning, and clinical leadership.

Job Summary

Coordinates the provision of direct patient care according to Nursing Care Standards.

The Registered/Graduate Nurse assesses, plans, implements, evaluates, and records patient care in accordance with the medical and nursing plans of care.

Participates in teaching and rehabilitation of the patient.

Coordinates discharge planning in conjunction with other members of the health team.

Directs and assists with the Licensed Practical Nurse, Nursing Assistant, and Nursing Student in rendering care to patients, works collaboratively with all members of the patient care team and provides leadership so that standards of professional nursing practice are maintained in the clinical setting.

Is able to respond quickly and appropriately to changes in patient's clinical condition.

Demonstrates ability to function as a charge nurse as assigned.

Demonstrates knowledge of the principles of growth and development over the span of life and identifies age specific needs and plans care accordingly.

  • Minimum Education Required
  • Must be a graduate of an accredited program of professional nursing.
  • Minimum Experience Required
  • 1-2 years med-surg experience preferred.
  • Licensure/Certification Required
  • Must possess current license as a registered nurse in the State of Pennsylvania or eligible nurse must pass their nursing boards within 6 months of hire.
  • BLS required.
  • Basic dysrhythmia course
  • ACLS
  • NIH Stoke Certification preferred or must be obtained within 6 months of hire.

Skills

Must possess nursing knowledge to effectively assess patients, formulate patient plan of care, implement and evaluate care and record completely.

Must possess interpersonal skills to effectively communicate with patients, family and other members of the health care team.

Must possess the necessary abilities to perform the essential functions of this position and complete the established orientation process.

Must demonstrate ability to utilize hospital computer system.

Must possess understanding and commitment to service excellence and performance improvement.

Requirements

Requires graduation from an accredited nursing program and a current Pennsylvania registered nurse license. Candidates should possess BLS, ACLS, and basic dysrhythmia certifications, with 1-2 years of med-surg experience preferred.
