This position will work with our multi-disciplinary team focused on class members served in the Williams Consent Decree. This program works towards moving individuals with severe and persistent mental illness/dual disorders who want to transition from living in a nursing facility to community settings. This position will provide a majority of services within the partnered nursing facility.

Candidates must hold a Master's degree in social work, human services, or a related field and be at least 21 years old. A valid driver's license, reliable transportation, and proof of automobile insurance are required.
