  • Degree: $60,000/annually + paid wellness day every other Friday

Your role

This position will work with our multi-disciplinary team focused on class members served in the Williams Consent Decree. This program works towards moving individuals with severe and persistent mental illness/dual disorders who want to transition from living in a nursing facility to community settings. This position will provide a majority of services within the partnered nursing facility.

Core responsibilities

  • Completion of assessments and making appropriate recommendations for persons served residing in the nursing facility
  • Collaboration with nursing facility staff to review records and gather information
  • Participation as a member of the multi-disciplinary clinical team

Knowledge, skills, and abilities

  • Ability to work within a collaborative, team-based approach
  • Strong communication and interpersonal skills to engage effectively with persons served, nursing facility staff, and clinical team members
  • Knowledge of mental health, substance use disorders, and community-based resources
  • Ability to conduct thorough clinical assessments and formulate appropriate recommendations

Qualifications

  • Master’s degree in social work, human services, or related field required
  • Minimum of 21 years of age
  • A valid driver’s license, reliable means of transportation, and proof of current automobile insurance required

Requirements
