The Registered Nurse performs nursing assessments, develops care plans, and monitors the health status of HCS program participants. They also supervise unlicensed staff, coordinate with healthcare providers, and ensure compliance with state regulations.
Candidates must hold an active Texas RN license and have at least one year of nursing experience, preferably in HCS or IDD/DD settings. A current CPR certification and the ability to pass a background check are also required.
The HCS Registered Nurse provides nursing services to individuals in the Home and Community‑based Services (HCS) program, ensuring person‑centered, high‑quality care in line with each Individual Plan of Care (IPC), state regulations, and professional nursing standards. The RN completes nursing assessments, develops and oversees care plans, trains and supervises direct support staff, and works closely with the interdisciplinary team. Location Humble and Kingwood, TX – Services provided in both office locations and in the homes of HCS clients. Perform initial and ongoing nursing assessments for program participants. Create, review, and update nursing service plans in collaboration with the interdisciplinary team. Monitor health status and respond promptly to changes in medical conditions. Administer medications and treatments or delegate tasks per Texas Board of Nursing guidelines. Train and supervise unlicensed staff in medication administration and health‑related tasks. Maintain accurate, timely documentation of nursing services, health updates, incidents, and reports. Participate in service planning and IPC meetings. Ensure compliance with TAC §9.176 and §9.178 and accepted nursing practices. Coordinate with physicians, pharmacies, and other healthcare providers. Conduct required on‑site visits to supported living, host homes, or foster care settings. Verify medical equipment and medications are current, stored safely, and used properly. Educate individuals and families on health management and wellness.
Active RN license in Texas. At least 1 year of nursing experience (HCS or IDD/DD preferred). Current CPR certification. Familiarity with HCS program rules and nursing delegation. Strong communication, organization, and documentation skills. Ability to work independently and manage time effectively. Must pass a background check and required screenings.
