The Transplant Social Worker provides pre-transplant patient education and emotional support from initial referral through the post-transplant period. They are responsible for coordinating patient care with multidisciplinary team members and facilitating referrals to community agencies.
Candidates must hold a Master's degree in Social Work and a current LCSW license. A minimum of two years of hospital or healthcare social work experience is required, along with STSW membership.
· Masters degree in social work. · STSW membership required in good standing · Certified Clinical Transplant Social Worker (CCTSW) credential is required within 3 years of joining Transplant Team · LCSW required Experience: · A minimum of two years of social work experience in a hospital or health care setting · Transplant social work experience preferred. · Excellent interpersonal and written communication skills · Current clinical assessment skills · Ability to act in an autonomous, self-directed manner.
