The RN Case Manager provides expert nursing care for home and facility-based hospice patients while serving as the primary clinical and communication lead. They coordinate care with interdisciplinary teams and families to ensure patient dignity and comfort.
Candidates must hold a current Texas RN license and have at least one year of nursing experience in hospice, home health, or acute care. A valid driver's license, reliable transportation, and strong organizational skills are required.

What You’ll Need Current Texas RN License Valid Texas driver’s license, reliable transportation, and auto insurance Graduate of an accredited Diploma, Associate, or Baccalaureate nursing program 1+ years of nursing experience (hospice, palliative care, oncology, home health, or acute care preferred) Ability to work Monday–Friday (8:30 AM – 5:00 PM) with rotating weekend on-call availability (full-time) Strong time management, organization, and documentation skills Experience with EMR systems (Consolo EMR a plus)

Preferred (Not Required) Bilingual (Spanish/English) BSN or higher Experience in hospice, home health, or case management Familiarity with Consolo EMR
