The Case Manager is responsible for developing, implementing, and managing patient care plans while coordinating continuum of care activities. They also facilitate linkages to community resources and ensure compliance with external agency requirements.
Candidates must have a high school diploma or equivalent and training in mental health or substance use disorder competencies. A valid driver's license is required, and the role involves working under the supervision of a licensed professional.
M-F 6a-2: 30p and possible Saturdays 6a-10a
