The Intake/Triage Nurse coordinates hospice referrals, manages physician orders, and facilitates communication between staff and insurance providers. They also perform phone triage, maintain patient scheduling, and provide clinical support to the hospice team.
Candidates must hold a specialized nursing diploma and possess at least one year of nursing experience in a healthcare setting. A valid Pennsylvania or New Jersey Registered Nurse license and BLS certification are required upon hire.

Associate’s Degree Nursing or Bachelor’s Degree Nursing 1 year home health/hospice experience or Less than 1 year nursing experience in a clinical office setting or Less than 1 year nursing experience performing phone triage CHPN - Certified Hospice and Palliative Nurse - Hospice and Palliative Nurses Association within 3 Years Physical Demands Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
