Home Care Nurse - RN - LPN

• Client is a 19-year old male with TBI, wheelchair bound, limited speech, hemiplegic

Available Shifts

• Park
• City: Every day of the week, 12 hour shifts days and nights, 7-7

We are looking for an RN or LPN who is interested in the following

• Medication set up and oversight
• Provide thoughtful care to clients based on personalized Plan of Care
• Provide in home assessments
• Monitor and track changes in the clients conditions and needs, and keeps the Director of Nursing and any other appropriate personnel informed
• Collaborate with client's family on plan of care, health changes, and concerns
• Weekdays include going to school with client
• Total personal cares

• RN and LPN Requirements
• Must have at least 1 year nursing experience
• Current unrestricted RN or LPN license in Utah
• Certified CPR Certification
• Negative TB skin test or chest x-ray
• Must be able to pass a drug test and a background check
• Licensed driver with valid automobile insurance
• Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. Must be able to lift 75 pounds
