The Home Health Aide provides personal care services, including assistance with daily living activities and companionship, under the direction of a nurse or therapist. They are responsible for documenting care, reporting changes in client condition, and maintaining a safe environment.
Candidates must hold a valid CNA license, nursing education, or equivalent competency certification and be at least 18 years old. A valid driver's license, insured vehicle, and current CPR certification are also required.

Job Summary Provides personal care services under the direction of the Registered Nurse or Therapist. The Aide is assigned to specific clients by the Registered Nurse or other appropriate professional who performs services for clients as necessary to maintain their personal comfort and well-being.
Provide companionship and, if authorized, transportation to clients according to the care plan. Assist client with general Activities of Daily Living (ADLs) such as bathing, meals, and safe ambulation/transfers. Report any changes in the client’s condition to the supervisor as outlined in the care plan. Document care provided and maintain accurate records for the Agency. Work collaboratively with the client’s care team and ensure effective communication. Promote client safety by following infection control practices and reporting unsafe conditions. Attend in-service programs to meet compliance and notify supervisors of any educational needs.
Successful completion of one of the following: A nursing education program preparing for RN or LPN licensure; Active CNA license with no restrictions; A nurse aide education program approved by the Virginia Board of Nursing; Current enrollment in a nursing education program preparing for RN or LPN licensure with at least one (1) nursing course completed that includes clinical experience involving direct client care; Completion of a competency evaluation program that meets the criteria of 42 CFR 484.36 (b). Home health aides need only be evaluated on the tasks in 42 CFR 484.36 (B) as those tasks relate tot he personal care services to be provided Be at least eighteen (18) years of age. Demonstrated ability to read, write, and follow a written care plan. Good verbal, written, and interpersonal skills. Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Current CPR certification.
