NOTE: This position requires driving from home to home, working with clients in the community, and transporting clients in personal vehicles.
How you will make a difference (job overview): In collaboration with a designated physician the Psychiatric Mental Health Nurse Practitioner (PMHNP) is responsible for providing outpatient psychiatric care, including intakes, assessments, and furnishing and providing medication management, following established standards and practices. The PMHNP will review and analyze participant records to furnish and evaluate the usage of psychotropic and other drugs and the quality and appropriateness of the care of mental health participants.
Division/Program
Overview: The program sees adults (18+) and older adults. It is a FULLY community-based program - all services happen at a person's home or community. The clients have complex needs - medication, housing, vocational, independent living and mental health. The intensity for services for each client is about 10 hours per month and once we are fully up and running, we'll have a census of about 50 adults.
