Inabia is looking for a Occupational Therapist to join our team in a Skilled Nursing Facility (SNF) setting. If you’re passionate about patient-centered care and enjoy collaborating with a supportive team, we’d love to meet you. You’ll help patients maximize independence with daily activities through evidence-based OT interventions and compassionate coaching. You’ll collaborate with the care team on goals, discharge planning, and caregiver education to support safe transitions
Key responsibilities
Perform evaluations, develop individualized care plans, and deliver therapy services based on patient needs.
Provide daily treatments, track functional progress, and support discharge planning.
Collaborate closely with therapy, nursing, and interdisciplinary teams.
Complete accurate, timely, and compliant clinical documentation.
Participate in skilled nursing rehab programming focused on functional recovery and safe transitions.
Contribute to a team-based approach that prioritizes patient outcomes and quality care.
SCHEDULE • Shift: days
Must be a graduate of an accredited Occupational Therapy program and hold a current state OT license. Strong communication, time-management, and patient education skills are required.
