The Social Worker serves as an integral part of the patient care team, facilitating discharge planning and coordinating referrals for community-based services. They also assist in organizing support groups and managing swing bed referrals while ensuring patient self-determination.

Responsibilities: required, but not limited to:

Serves as an integral part of the patient care team working with providers, nursing, and other clinic staff as well as the patient and their supports.

Ensures patient self-determination in decision making.

Help patients, families, and outside agencies in the development of appropriate discharge plans that meet the social, medical, economic, and emotional needs of the patient.

Assists in making appropriate referrals for patients such as long-term care, home care and community-based services.
This includes follow-up and communication with other service providers.

Working knowledge of area resources and processes for obtaining these services for patients.
10.
Serves as an active team participant in Utilization Review and the discharge planning process.
11.
Assists in planning and conduction of programs to assist patients with social problems and improve community health and counseling services.
12.
Assists in organization of support groups for patients and family members.
13.
Serves as the primary contact for swing bed referrals both internally and externally.
Assists in completing initial paperwork as well as coordination of multi-disciplinary team meetings to monitor progress while in the hospital setting.
Assists in making appropriate referrals for services upon discharge from swing bed.
