We are seeking a Hospice Nurse Case Manager to be responsible for identifying patient/family needs and for providing skilled nursing assessment, planning and supportive care and appropriate services in accordance with the attending physician’s plan of treatment and orders.
Candidates must hold a current California Registered Nurse license and have at least one year of professional nursing experience within the last three years. A Bachelor's degree in Nursing is preferred, as is experience in hospice or home health settings.
