The nurse will deliver direct patient care in home environments, manage care plans according to physician orders, and maintain accurate medical records. They will also coordinate with healthcare team members and provide emotional support to patients and their families.

Deliver direct patient care as assigned, using established procedures in the patient’s home environment.
Traveling to patients’ homes and managing their care plans according to physicians’ instructions
Maintain accurate, detailed reports and records on medical information, vital signs, or changes in patient’s conditions.
Monitor patients' medication usage and results.
Document patients' medical and psychological histories, physical assessment results, diagnoses, treatment plans, prescriptions, or outcomes
Evaluate patients' behavior to formulate diagnoses or assess treatments.
Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans.
Provide personal assistance, medical attention, emotional support, or other personal care to patients.
Coordinate with other home health professionals caring for the patient, and communicate the patient’s needs to the family providing support.
Train other Nurses, coordinate with other home health professionals caring for the patient, and communicate the patient’s needs to the family providing support.
Assures the continuity of care through the delivery of quality patient/client care.
Provides effective communication to patient/client/family, team members, and other health care professionals as evidenced by clinical notes, case conferences, and communication notes. Completes appropriate documentation in a timely manner
Monitors assigned cases to ensure compliance with physician orders and requirements of third party payor.
Demonstrates commitment, professional growth, and competency.
Other duties as assigned.
