Candidates must have at least 2 years of acute care hospital Case Manager experience, including proficiency in InterQual and Epic EMR. A valid California RN license is required, along with proven experience in care coordination and discharge planning.

This role supports an acute care hospital case management team focused on utilization review, care coordination, and discharge planning for adult and geriatric patients.
POSITION OVERVIEW This assignment is ideal for experienced inpatient Case Managers who are confident managing high caseloads, coordinating care across interdisciplinary teams, and ensuring appropriate utilization of healthcare resources in a fast-paced acute care environment.
