KeystoneCare is seeking compassionate and dedicated Hospice On-Call Registered Nurse to provide weekend support for our hospice patients and their families.
This critical role ensures that patients receive expert care and guidance during off-hours, helping to manage symptoms, provide comfort, and support families through the challenges of end-of-life care.
As an on-call nurse, you will be a vital member of our interdisciplinary hospice team, responding to urgent patient needs, collaborating with physicians and staff, and assisting with care coordination to ensure seamless service delivery.

Assess patient conditions, manage symptoms, and provide necessary nursing interventions.
Provide guidance and support to patients' families and caregivers over the phone or in person as needed.
Collaborate with the hospice interdisciplinary team to ensure continuity of care.
Document all patient interactions, assessments, and interventions accurately and timely.
Adhere to hospice philosophy and policies in all aspects of care delivery.
Admit patients who are hospice appropriate and interact with families in assessing needs and treatment plan.
