The Job We are seeking talented individuals to help drive the success of adults in our communities. To provide leadership for multiple treatment teams in facilitating a successful return of adults to their home, independent living, and/or community; to advocate for our adults, families and maintain excellent relationships with outside agencies including courts, referring organizations, schools, and community resources.

• Snapshot of Daily Duties Follow up with Admissions Coordinator to ensure consent forms are signed and update signatures to maintain compliance.
• Orientate the clients to SCOA expectations and complete intake packet.
• Act as a liaison for SCOA and outside agencies including schools and community.
• Update client info in database on intake and throughout client’s stay at SCOA.
• Keep accurate documentation of Agency/Family interactions and maintain in chart and in database. (Including satisfaction surveys) The Must Haves Minimum of a high school diploma or GED is required Over the age of 21 Maturity, empathy and patience Be special – Not everyone can do what we do The ideal candidate must have the ability to adjust to a fast-paced environment and follow proper protocols
