The Occupational Therapist will perform home health evaluations, develop individualized care plans, and provide ADL training to patients in their homes. They are responsible for accurate OASIS documentation and managing an independent caseload within a designated territory.
Candidates must have at least one year of skilled home health experience and proficiency with OASIS documentation. A valid Oregon Occupational Therapy license and BLS certification are required, along with reliable transportation for field travel.

This role supports a home health program delivering one-on-one patient care focused on improving independence, functional mobility, and activities of daily living in the home setting.
POSITION OVERVIEW This assignment is ideal for Occupational Therapists with strong home health experience who are confident working independently, managing productivity expectations, and completing OASIS documentation accurately.
