This role supports a home health program delivering one-on-one patient care across a defined territory, focusing on rehabilitation, mobility, and independence in the home setting.
POSITION OVERVIEW This assignment is ideal for experienced Home Health PTs who are confident working independently, managing their own caseload, and completing OASIS documentation accurately and efficiently.
