The physical therapist will perform home health evaluations, develop individualized care plans, and provide rehabilitation services to patients in their homes. They are responsible for completing OASIS assessments and documenting all care within the Epic system.
Candidates must have at least one year of skilled home health experience and proficiency with OASIS documentation. A valid Oregon Physical Therapy license and BLS certification are required, along with reliable transportation for field work.

This role supports a home health program delivering individualized patient care across a defined territory, focusing on rehabilitation, mobility, and functional independence in the home setting.
POSITION OVERVIEW This assignment is ideal for Physical Therapists with strong home health experience who are comfortable working independently, managing their own caseload, and delivering care across a variety of home environments.
COMPENSATION Because travel rates change frequently based on market demand and facility needs, our recruiters provide the most current and competitive pay packages during your initial conversation. Each compensation package is customized and may include taxable hourly wages along with tax-free stipends when applicable.
