The RN Case Manager coordinates care and performs continuous assessments of hospice patients' physical, medical, psychosocial, and spiritual needs. They work as part of an interdisciplinary team to implement and manage the patient's plan of care.
The role requires a registered nurse who is committed to hospice care and professional collaboration. Candidates must be able to provide expert care and support to patients and families facing terminal illness.
