The Case Management Coordinator assesses and evaluates the needs of individuals to develop strength-based, person-centered treatment plans. They also provide advocacy, coordinate community resources, and maintain clinical documentation to support client recovery.
Candidates must possess a Bachelor's degree or higher in a relevant field along with specific supervised experience in the treatment of minors and their families. Additionally, applicants must hold a valid professional license in the State of Michigan and meet CMHP guidelines.
