Job description

The Responsibilities\nRound at assigned facilities and manage the acute, subacute, and primary care needs of this fragile population.\nWork closely and collaboratively with the OnSite team, nursing home staff, residents, and families.\nProactively follow up with patients and assess the efficacy of recommended interventions.\nProvide education on best practices.\nIntegrate various interventions, emphasizing non-pharmacological treatments whenever possible.\nMaster of Science in Nursing with FNP, ANP, AGNP, AGACNP, AGPCNP, or ACNP certification\nActive License in South Carolina\nDEA registration\n1+ year experience\nExcellent communication and documentation abilities.\nStrong patient assessment skills.\nStrong, ambitious, patient-centric work ethic.\n\n\nBenefits\nMonday - Friday visiting schedule with flexibility\nTraining \nMedical/Dental/Vision\nLong and Short-Term Disability\nLife Insurance\n23 Days PTO\n5 CME days\nLicensing reimbursement\n\n\n#ONS
