The Case Manager assists individuals with mental health conditions in developing social, occupational, and living skills to live successfully in the community. Responsibilities include coordinating care with clinical teams, providing crisis de-escalation, and linking consumers to essential medical and social resources.
A bachelor's degree in Human Services or a related field is required, along with a valid Colorado Driver's license and a clean driving record. Candidates should possess strong clinical, organizational, and communication skills, with Spanish bilingualism preferred.
The Case Manager is an integral member of Community Reach Center’s Outpatient, Community programs, and Case Management Teams. The Care Manager role will emphasize assisting persons to explore and develop social, occupational, leisure, and living skills. Provides services that supports consumers in living in the community successfully including linking consumers with resources to obtain and/or learn basic needs. Essential Functions: Provides prevention/early intervention, case management, family psychoeducation, and other clinical interventions to consumers and families Support and closely coordinate mental health care with the consumer’s clinical care team and the primary care provider Attend collaborative treatment team meetings and reviews of consumers Provides crisis de-escalation when needed Assist eligible consumers in gaining access to needed medical, social, educational, and other services Provide an assessment of needs and service plan development Empowers consumers to reach their goals and sets a positive example and provides role modeling as well as technical expertise to accomplish goals Provide transportation for consumers in agency vehicles when needed Will work closely with outside organizations on resources and care coordination Using an evidence-based model of measurement-based care tracks success for each individual consumer
Requires bachelor’s degree Human Services or related field Communication, organization, time management and clinical skills. Ability to work independently in a community-based setting. Valid Colorado Driver’s license with a clean driving record. Bilingual Spanish preferred, but not required Physical Requirements: Regularly required to, stand, sit; talk, hear, and use hands and fingers to operate a computer and keyboard. Specific vision abilities required by this job include close vision requirements due to computer work. Light to moderate lifting is required, Regular, predictable attendance is required.
